I authorize Micro Mage Repair LLC to perform repair work on my device.
I understand that Micro Mage Repair LLC is not an authorized service dealer.

I agree to release and indemnify Micro Mage Repair LLC from liability for any claims or
damages of any kind or description that may arise from any repair work performed on my
electronic device.

I will not hold Micro Mage Repair LLC responsible for any data or software loss which may occur as
a result of existing damage or attempts to repair existing damage on my device.

Micro Mage Repair LLC will not browse through any personal, private, or confidential
information or data unless the device is in for data recovery services specifically.
I understand a requirement of data recovery services is confirmation of successful recovery by
Micro Mage Repair LLC. I understand that any confidential data is my responsibility to back up or remove prior to service.

I understand that repairs or technical support performed by Micro Mage Repair LLC
may void manufacturer warranties, and that it is my responsibility to know if my device’s
warranty may be invalidated by independent repair service attempts.
Micro Mage Repair does not assume any liability or warranty in the event that the manufacturer
warranties are voided, but may, at their sole discretion, offer their own warranty on parts provided
and/or services performed.

I understand a fee may be assessed for undisclosed prior repair attempts at the sole discretion of
Micro Mage Repair LLC.

I understand if I do not provide a pass code, the complete results of the repair will be rendered
unknowable to MicroMage Repair LLC on some devices , and that warranty on a repair that was
not fully tested will not be provided. (Devices requiring passcodes to complete the repair will be
disclosed prior to repair attempts.)

 

I understand Micro Mage Repair LLC is not responsible for damage incurred during shipping to or 

from our facilities and that such damage should it occur will not be covered under our warranty policy.

I understand that under Colorado law, failure to pay within 30 days of first request may result in
my device being sold in whole or part by Micro Mage Repair LLC to recover losses in time and parts
incurred by payment failure with no more or less than 15 days notice.

FILLING OUT THE FORMS BELOW CONFIRMS THAT I HAVE READ, UNDERSTOOD, AND AGREE TO BE BOUND
BY THESE TERMS AND CONDITIONS.
